Subject:De- empanelment of HCOs under Continuous Empanelment Scheme under CGHS, Chandigarh (HQ) – regarding
With reference to the request letter received on 15/12/2020 from the Apollo Specialty Hospital, Amritsar regarding termination and discontinuation of Agreement dated 27/07/2020 with CGHS; as per the terms and conditions of the MOA para 25: the Mandatory one month notice has lapsed on 14/01/2021, thus the termination and discontinuation of the hospital is confirmed w.e.f 18/01/2021.
