Sub: De-empanelment of Tricolour Hospital, Vadodara reg.
This is with reference to written request received from Empaneled HCO Tricolour Hospital, Vadodara, Dt- 30/09/2021.
As per MoA & production of One month advance notice; Tricolour Hospital, Vadodara is hereby De-empaneled from the empaneled HCOs list of CGHS Ahmedabad w.e.f 01/11/2021. However, Tricolour Hospital, vadodara is instructed to provide treatment to the admitted CGHS beneficiaries till completion of their treatment.
