In pursuance of the Ministry’s OM F. No. 5-34/CGHS/HEC/HQ/2025 dated 22nd December 2025, Avalon Hospital (A unit of Hemendra Smriti Health Care Pvt Ltd) Paribahan Nagar, Matigara Dist-Darjeeling, Siliguri – 734010 is empaneled under CGHS Kolkata with effect from 05th June 2026 for a period of 3 (three) years as per the Terms & Conditions of empanelment laid down in the Memorandum of Agreement.
