In pursuance of the Ministry’s OM F. No. 5-34/CGHS/HEC/HQ/2025 dated 22nd December 2025, KOSMODEN Dental Clinic, Plot No. 412 (R.S) & 566 (I.R) N.C. Complex, Opp. Narsingha School, Shiv Mandir, Matigarha, Siliguri- 734 011 is empaneled under CGHS Kolkata with effect from 09th June 2026 for a period of 3 (three) years as per the Terms & Conditions of empanelment laid down in the Memorandum of Agreement.
