No. 5-34/CGHS/HEC/HQ/2025 dated 22nd December 2025, CHARAK Square Hospitech 79, Panchanantala, PO – Chaltia, Vill – Chaltia (ct), Block & PS- Berhampore, Dist – Murshidabad – 742102, West Bengal is empaneled under CGHS Kolkata with effect from 16th October 2026 for a period of 3 (three) years as per the Terms & Conditions of empanelment laid down in the Memorandum of Agreement.
