OFFICE MEMORANDUM
Subject: Fresh Empanelment of Private Hospital/HCOs under CGHS Patna.
With reference to the subject cited above, and in accordance with the applicable package rates notified vides Office Memorandum (OM) No. F. No. 9-16/CGHS(HQ)/HEC/2024 (Part-I) (Comp No. – 8365027) dated 03.10.2025, as well as the guidelines issued for Fresh Empanelment of Private Hospitals under CGHS vide Office Memorandum No. F. No. 5-34/CGHS/HEC(HQ)/2025 (Comp No. 8375449) dated 22.12.2025, the following Health Care Organization (HCO) has submitted a duly signed Memorandum of Agreement (MoA), Hospital Policy for CGHS beneficiaries, and a Performance Bank Guarantee to this office on 27.03.2026.
Accordingly, the following Exclusive Dental Clinic is hereby empanelled under CGHS Patna with effect from 30.03.2026 for a period of three (03) years. The empanelment shall cover all CGHS-approved and available facilities at CGHS-notified rates.
