Subject: Fresh Empanelment of Private Health Care Organisations (HCOs) under the administrative control of CGHS Bhopal – reg.
Ref: Ministry’s O.M. F.No.5-34/CGHS/HEC (HQ)/2025 dated 22.12.2025
In Compliance to the Office Memorandum issued by the Directorate, CGHS, Ministry of Health & Family Welfare (MoHFW), dated 22/12/2025, approval is hereby accorded for the empanelment of the following Health Care Organisations (HCOs) with effect from the date mentioned against their names:-

The empanelment shall be valid for a period of three (03) years from the date of issue of this order.
The empanelled Health Care Organisations shall strictly comply with all the terms and conditions stipulated in the aforesaid Office Memorandum dated 22/12/2025. Any deviation or non-compliance with the prescribed terms and conditions will invite appropriate action, including De-Empanelment of the HCOs, as per the provisions contained in the said Office Memorandum
